Yes the message is overlayed in the RGB elements of random pixels. So that it appears in the image as just a subtle pixel colour change indistinguishable from all the other pixels. Only the text of the message is inserted along with a simple message terminator character sequence which, like the rest of the message, is indistinguishable from the noise of the image without access to the insertion profile.

False positives, decoy messages, can easily be inserted just by varying the steganography profile.

So long as the areas in the steganography profiles do not overlap then they is no limit the the number of individual messages that can be inserted in an image. There is also no way to tell how many messages are overlayed in an image.

5. Why can't the output be a Jpg file?

Jpg files use a compressed data format. This process alters the pixels RGB values in the images destroying any message that has been inserted.
